You authorize that Northwoods Pet Care Cntr can provide necessary emergency care for your pet. If your pet has fleas, Northwoods may apply appropriate medication to your pet to stop transmission.

You authorize that you are responsible for all debts acquired at NWPCC and that payment is due at time of service.

Northwoods Pet Care Center is not liable for any injuries that may occur while your pet is here. Likewise, you will not be liable for any staff member injured by your pet.
