New Client Form

PAYMENT/BILLING: Full payment is due at the time services are rendered. We do not bill. Any account with an unpaid balance, will accrue monthly interest. We accept all major credit cards and Care Credit. Personal checks are accepted with a valid ID. We also pair with Trupanion pet insurance for your convenience. In addition clients may place a credit on their account prior to coming in, to help cover any and all future medical expenses. By signing this form on the last line, you are hereby authorizing that you will be the individual whom is financially and legally responsible for all debts acquired at Northwoods Pet Care Center. You also agree that you are above the age of 18 years, and give the staff at Northwoods Pet Care Center the authority to examine, prescribe for, or treat the pets described below, and any additional pets within your account.
